Position Overview

We are seeking a compassionate and skilled Social Worker to provide support to clients and their families in navigating the challenges of healthcare, mental health, and social services. The ideal candidate will empower individuals by offering guidance, advocacy, and access to the resources they need to improve their quality of life.

Key Responsibilities

• Assess the needs, strengths, and challenges of clients through interviews and evaluations.
• Develop and implement individualized care plans tailored to client needs.
• Provide counseling and emotional support to clients and their families.
• Advocate for clients in accessing healthcare, housing, financial assistance, and community resources.
• Collaborate with multidisciplinary teams to ensure holistic care for clients.
• Maintain accurate and confidential client records and documentation.
• Conduct crisis intervention and support in emergency situations.
• Stay up-to-date with community resources, policies, and best practices in social work.

Qualifications and Requirements

• Bachelor’s or Master’s Degree in Social Work (BSW/MSW) from an accredited institution.
• Registration with a relevant professional regulatory body (e.g., Ontario College of Social Workers, or equivalent).
• Minimum of 3 years of experience in social work, preferably in healthcare or community services.
• Strong knowledge of community resources, social service programs, and advocacy techniques.
• Excellent communication, problem-solving, and organizational skills.
• Ability to work independently and collaboratively in a multidisciplinary team.
• Proficiency in case management and maintaining detailed records.
• Compassionate, patient, and dedicated to client well-being.
